Care & Connects is an innovative NDIS provider who is dedicated to delivering the best quality support to each of our participants.  To assist with this, we have a dedicated Psychosocial Disability program led by experienced Mental Health professionals, delivering Supported Independent Living (SIL) and Community Participation (CP) supports across the Adelaide region.

About you:

We are looking for compassionate individuals with a genuine desire for supporting individuals with specific conditions and needs. You possess a strong understanding of the NDIS framework and are dedicated to its principles.

 

Qualifications

  • Minimum Certificate III / IV in Disability / Mental Health or equivalent.
  • Medication Administration Accreditation
  • NDIS Worker Screening
  • Manual Handling Training
  • First Aid and CPR
  • CPI/MAPA/Mental Health First Aid is desirable
  • Any additional trainings such as PEG/Bowel/Diabetes is an advantage
  • 100 Points ID

 

Selection Criteria

  • Availability to work in the following location and times:
    • SA 5169:
  • Minimum 1 year in high care and/or mental health support, including but not limited to bipolar disorder, anxiety, schizophrenia, intellectual disability, challenging behaviours, high needs care, etc.
  • An understanding of the impact chronic pain, trauma, grief and loss has on an individual and their families, particularly those with complex needs.

Shift Timings:

Morning- 7:00 AM- 3:00PM or 6:00 AM- 2:00 PM

Noon - 3:00PM- 11:00 PM or 1:00 PM- 5:00 PM

Night: 5:00 PM -10:00 PM +Passive night +11:00 PM-7:00 AM

 Your tasks may include but not be limited to the following:

  • Providing personalized care and support to NDIS participants, including personal hygiene; meal preparation; medication; and activities of daily living.
  • Implementing positive behaviour support plans and crisis intervention strategies
  • Provide emotional and practical support to participants according to their individual needs.
  • Monitor participant’s progress and report any changes or concerns as required
  • Maintain accurate records and documentation in line with NDIS and organisational requirements
