Care & Connects is an innovative NDIS provider dedicated to delivering the best quality support to each of our participants. To assist with this, we have a dedicated Psychosocial Disability program led by experienced Mental Health professionals, delivering Supported Independent Living (SIL) and Community Participation (CP) supports across the Adelaide region.

About you:

We are looking for an experienced Support Coordinator with a strong understanding of the NDIS framework, and its principles. As a Support Coordinator, you will have a crucial role in providing comprehensive support coordination services to our participants.

 

Desired Qualifications

  • Bachelor’s in social work, Psychology, Allied Health, Social Science or other relevant fields and registration with a relevant professional body (AASW/ACWA or similar) is mandatory
  • Minimum 2 years of experience in Support Coordination within the NDIS environment is preferred.
  • Experience in Psychosocial Recovery Coaching and Level 3 Support Coordination is desired but not mandatory.
  • Freshers are encouraged to apply.

 

Selection Criteria

  • Experience with complex case management and ability to develop strategies to address barriers and points of crisis within a multi-disciplinary team.
  • Experience implementing plans, monitoring and follow-up to negotiate appropriate support solutions with multiple stakeholders and seeking to achieve well-coordinated plan implementation.
  • Understanding and respecting the participant’s privacy and balancing this with information shared with the community and mainstream providers.
  • Understand the delineation of roles and responsibilities between the NDIS and Corrective Services, particularly around risk.
  • Previous experience in community services, disability services, mental health services, or similar is preferred.

 

Your tasks may include but not be limited to the following:

  • Assist participants in successfully coordinating their support needs and in achieving outcomes of their NDIS plan
  • Provide support coordination within the agreed timeline and funds.
  • Provide advice to participants and their carers on the evolving environment of the NDIS
  • Research and identify providers and other relevant stakeholders and build strong working relationships, prepare reports, monitor expenditures, and complete all associated administrative work
  • Develop support plans to address barriers
  • Negotiate appropriate support solutions with multiple stakeholders and seek to achieve well-coordinated plan implementation.
  • Liaise with NDIA, NDIS Quality and Safeguards Commission, and other key stakeholders as required.
  • Ensure all mandatory reporting is undertaken and required assessments completed within timelines and of high quality.
  • Ensure that all funding management activities are undertaken and that participants can maximise outcomes outlined in individual plans.
  • Assessing the needs and coordinating support for NDIS participants and setting budgets.
  • Identifying potential improvements to the existing services and processes.
  • Work independently in any challenging environment.
  • Maintaining case notes.
  • Educate participants on the NDIS plan and funding structure.
  • Ensuring adherence to company policies and procedures.
  • Research, coordinate and manage a range of varied levels of support across multiple providers, to suit participant's need.
  • Actively develop, implement, monitor, and review participant's NDIS plans and report on progress and outcomes
