To assist with this, we have a dedicated Psychosocial Disability program led by experienced Mental Health professionals, delivering Supported Independent Living (SIL) and Community Participation (CP) supports across the Adelaide region.

About you:
We are looking for an experienced Team Leader with a strong understanding of the NDIS framework, and its principles. As a Team Leader, will provide leadership and support on day-to-day basis to the support workers who provide care to our participants in their Supported Independent Living arrangement. This position is ideal for a professional who is passionate about making a difference and has the expertise to lead a dedicated team in delivering exceptional support services
Qualifications
- Bachelor of Nursing / Certificate IV in Disability or Equivalent
- Medication Administration Accreditation
- NDIS Checks
- Manual Handling Training
- First Aid and CPR
- CPI/MAPA/Mental Health First Aid is desirable
- Any additional trainings
Selection Criteria
- Experience of minimum 1 year as a Team Leader
- Demonstrated proficient written, verbal, and interpersonal communication skills.
- Demonstrated ability to work with minimal supervision, and collaboratively as a member of a multidisciplinary team.
- Ability to effectively prioritise and organise own work / caseload.
- Ability to work with any complex need’s client.
- The role requires you to work from the client location 4 days & 1 day at our office premises.
Your tasks may include but not be limited to the following:
- Overseeing the daily operations of the SIL house & Community residents
- Working with a range of complex participants, including those with severe and persistent mental conditions, and/or multiple diagnoses
- Monitoring & mentoring the support workers to maintain workplace health and safety checks, general operations, and cleanliness of the property.
- Providing exceptional & consistence service to participants/ residents in the SIL house & Community
- Leading the team through routine supervision and conducting ongoing team-briefing/preparation meetings regarding different levels of personal care, transport service, community, and domestic duties
- Implementing positive behavior support plans and coaching support workers in implementing the same
- Regular overseeing of restricted practice audit tools and medication practices
- Ensuring that all support workers are trained and up to date with relevant procedures and regulations.
- Proactively identifying and conducting risk assessments, timely response and providing decision-makers with information of the same to avoid risks that will adversely affect safety and well-being of the SIL residents.
- On-going supervision and training of support workers on administrative tasks related to daily reporting procedures, documentation, and incident management.
- Investigating client and/or clinical issues and problem-solve to achieve realistic and practicable solutions.
- Participating in regular team meetings, client briefings and professional development activities as when required by Care and Connects
- Manage Reporting Folder which includes generation, updating and time to time maintenance of reporting
- Special Focus on reporting to Management which could be Weekly, Fortnightly or whenever required depending on participant or service agreement
- Being available for On-call duty on a rotational basis/after hours or as required by Care and Connects
