To assist with this, we have a dedicated Psychosocial Disability program led by experienced Mental Health professionals, delivering Supported Independent Living (SIL) and Community Participation (CP) supports across the Adelaide region.

About you
As a Service Coordinator, you will be responsible for delivering high-quality, person-centred services that align with the participant's needs and preferences under the NDIS framework. This role involves coordinating participant onboarding, developing individualised support plans, monitoring service delivery, and working close with internal teams, families, and external stakeholders.
Your tasks may include but not be limited to the following:
- Conduct participant assessments and develop individualised support plans aligned with their goals and needs.
- Coordinate onboarding and ensure all documentation is accurate, compliant, and up to date.
- Arrange and monitor service delivery, ensuring quality, effectiveness, and timely supports.
- Support participants to exercise choice and control while promoting independence and social inclusion.
- Respond to incidents and risks, ensuring participant safety and compliance with reporting requirements.
- Build and maintain strong relationships with families, providers, and stakeholders.
- Ensure compliance with NDIS standards, organisational policies, and quality frameworks.
- Provide guidance and support to staff, contributing to a positive and high-performing team environment.
- Being available for On-call duty on a rotational basis/after hours or as required by Care and Connects
Selection Criteria & Qualifications
- Experience of 3 years in disability services as a Service Coordinator or similar role.
- Strong understanding of the NDIS framework, Practice Standards, and Code of Conduct.
- Ability to manage multiple participants and priorities effectively.
- Demonstrated proficient written, verbal, and interpersonal communication skills.
- Diploma or Certificate IV in Disability, Community Services or a related field.
- Medication Administration Accreditation
- NDIS & WWC Check
- Manual Handling Training
- First Aid and CPR
- AU Driver's licence
- Any additional trainings
