About the Role:
In this dynamic position, you’ll provide comprehensive healthcare support to participants in line with NDIS guidelines. You’ll oversee clinical care, medication compliance, and risk mitigation while leading a team committed to delivering high-quality, participant-centered care. Key responsibilities include conducting health assessments, developing care plans, delivering training, and collaborating with internal and external stakeholders to achieve exceptional health outcomes.

Key Responsibilities:

  • Conduct detailed health assessments and develop individualised care plans.
  • Provide leadership to support workers, ensuring best practices in clinical care.
  • Deliver health-related education and training to participants and caregivers
  • Develop, implement, and regularly review individual care plans in collaboration with participants, their families, and Management.
  • Monitor participant progress and compliance with NDIS standards.
  • Respond to clinical incidents and manage risk mitigation strategies effectively.
  • Collaborate with multidisciplinary teams to enhance care quality.

About You:

  • Bachelor of Nursing - Registered Nurse with AHPRA registration.
  • Minimum of 3 years’ experience in aged care or disability services.
  • Strong leadership, communication, and interpersonal skills.
  • Commitment to excellence in clinical care and continuous improvement.
  • Valid First Aid/CPR, WWC, NDIS Worker Screening, Police Check and Australian Driver’s License.
  • Availability - Monday to Friday 9am to 5pm.
  • Available to work in both Salisbury and Prospect Offices.
